Absence Request form

As you know, attendance is essential to our success in the Woodland Band. We expect all students to attend all rehearses as this benefits the band AND the student. The band calendar is posted with many weeks notice and we do our best not to make changes. When we have to add things, we try to give as much notice as possible to ensure that all of our students are at rehearsal. 

In the event that you need to miss a rehearsal, this form must be completed at least one week prior to the anticipated absence. If you have an absence due to an emergency, fill out the form as soon as you can regarding the absence. Unexcused absences may result in being sidelined for a marching band performance, loss of full-time performance spot, and being ineligible for a Varsity Band Letter. 

You will only receive a reply to a submission should there be a problem. By submitting this absence request, you hereby sign your name to the information on this form verifying its authenticity and fidelity. 

               Excused absences include but are not limited to:
                   1.  Sickness (doctor's note expected when this is the case)
                   2.  Death in the family

               Unexcused absences include but are not limited to:
                   1. Homework or projects
                   2. Work
                   3. Medical appointments (we ask that you make these appointments around our rehearsal and performance schedule)
                   4. Being tired
                   5. Forgetfulness
